Support Custom Marker Types in Index

Summary

Add support for FrameMaker custom marker types in HTML Help indexes per http://wiki.webworks.com/DevCenter/Projects/Custom_Index_Markers.

Detailed Description

FrameMaker allows custom marker types to be defined, allowing the generation of separate indexes for each marker type in a document set. This can be used to flag different semantic entities and compile separate alphabetized lists of each entity for printed output.

When converting such docsets to HTML Help, WWeP should include an option that permits custom marker types to be treated as Index markers, so their contents appear alongside the standard index entries on the Index tab of the HTML Help (CHM) file.

Currently, these entries are lost when output to HTML Help, since WWeP ignores the custom markers.

Note: It is possible to pre-process the FrameMaker files to convert all custom marker types to Index markers before WWeP conversion (and restore the custom marker types afterwards), but this solution is error-prone, introduces an additional level of complexity and requires third-party scripting tools and expertise that may not be available to all users.

Earlier WWP versions did not require this hack, and it would be very helpful if WWeP could restore support for this useful feature.
